Follicle Has White Stuff after Hair Transplantation

Our patient from our Orange County Hair Transplant Surgery office wrote in regards to his recent hair transplant procedure. It was regarding the “white stuff” that we left around the hair follicle after the procedure. He had looked up other hair transplants and did not see any other patients with his situation and was worried this might not be normal.

In our facilities we practice special techniques to prevent post-op complications. A hair transplant deals with more than just hair. It is an entire organ consisting of a microscopic muscle, collagen band, supporting vessels and epithelium (superficial layer of skin). The white portion that is seen is the epithelium of the hair graft. The main objective is not to bury the hair graft during surgery to prevent possible complications. We make sure to keep the hair graft slightly elevated from the surface of the skin to prevent these complications. This particular technique can in fact help prevent common post-op issues including folliculitis, sebaceous cysts and/or dimpling of the skin.

After a short period of time the white part will dry out and fall off either by itself or through regular washing of the hair. Little techniques like this one can make a vast difference for our patients by minimizing worry and severe issues.

Comparative Study Between Radiesse Juvederm Perlane Fillers

Dermal Fillers and Injectable TreatmentsA recent article regarding a comparative study between major dermal fillers available in the US market caught our attention when we received it in out Hair Transplant Surgery and MedSpa in Orange County. These fillers included Juvéderm, Juvéderm ULTRA, Perlane, and Radiesse. The study benefits patients concerned about which filler would probably be best for their personal use and can also help physicians assist their patients in that choice process. Two standardized scales were used to make their assessments on the products: the Global Aesthetic Improvement Scale (GAIS) and the Wrinkle Severity Rating Scale (WSRS).

The rising popularity in dermal fillers like Radiesse, Juvederm, and Perlane  has raised questions from patients and physicians. This particular study was designed to help patients and physicians make more educated choices on fillers for the best results with the least overall expense. A panel of 205 patients between the ages of 18 – 80 years of age (52 being the median) were randomly administered an adequate amount of an undisclosed dermal filler (Radiesse, Juvéderm, Juvéderm ULTRA, or Perlane) to produce the fullest result. Touch up was scheduled within the next 4 months they were to be assessed when they return at 4, 8, and 12 months after for reevaluation.

4 months after initial treatment, all patients returned and received a touch and discussed their satisfaction level before reapplication. Of the 4 filler types, Radiesse patients reported the highest level of satisfaction. Regular Juvéderm received the lowest level of satisfaction.

When patients returned at 4 and 8 month reevaluation periods, all patients that received Radiesse had reported nearly the same level of satisfaction with their results. Perlane and Juvéderm ULTRA did better by this point but Juvéderm required reapplication in most patients. By 12 months, Radiesse had the highest patient satisfaction with the least application or reapplication. Juvéderm received the lowest with the highest units of reapplication.

Visibility of the patient’s Nasolabial Folds (NLFs) was the determining factor in patient satisfaction. All products used did exactly what they were designed for. Some required more of the product to achieve adequate fullness. Below is a median table summarizing the units (syringe count) total applied on patients:

• Juvéderm – 8 Syringes
Juvéderm ULTRA – 4 Syringes
Perlane - 3 Syringes
Radiesse - 2 Syringes

Can I do Martial Arts after Hair Transplant?

A patient of ours brought up a good question regarding physical activities. He is very physically active and performs mixed martial arts as well as competes. He asked, “…do you think my head will be ok if I’m doing that? Is it high risk for new transplant hair or donor scar…”? At this time, he just had surgery and his competition is in the next 3 weeks.

In regards to the transplanted hair, the hair grafts are safely implanted in their new area after 4 days from hair transplant surgery. The donor scar is still healing but will be sealed within 2 weeks of surgery. By the time our patient needs to compete the donor scar and recipient areas are healed. The only concern is the chance of stretching or widening the donor scar due to excess tension or over-stretching.

Many regular physical activities should not cause any alarm to the patient but some contact or heavy sports can be concerning. We suggest giving the donor area a minimum of 1 month to heal enough to continue with most sports and training. That should be enough time to give the scar some of its resiliency. We in our Hair Transplant Surgery office in Orange County suggest waiting at least 3 months after surgery to do anything really strenuous but at this point we leave it up to our patient’s own discretion.

Maintenance of Hair after Transplant

“When can my son start handling his hair after his hair transplant?”  One of our patient’s concerned parents emailed our Orange County Hair Transplant clinic this question and we wanted to post our answer.  Hair transplant surgery raises many questions like this one from time to time and we make sure to answer them for all our patients.

Hair restoration surgery is a minimally invasive procedure and requires little maintenance after the procedure is completed.  We make sure to clean the treated area the day of surgery and teach our patients how to wash their hair the following day.  The most crucial time after surgery is the first four days of surgery and the hair needs to be treated and cleaned carefully.  By day five the treated areas are completely settled and you can go back to maintaining your hair and the rest as you would normally.

The only limitation most hair transplant surgeons have for their patients is in regards to excessive sun exposure.  We recommend avoiding over exposure for at least 6 months after surgery but if the patient needs to be in the sun we suggest wearing sunscreen, a hat or head cover (bandanna). In regards to concealing the transplanted area, it can be done by changing your hair style a bit or by covering the area with a hat as well.

Hairline Lowering for Women through Hair Transplantation

A patient of our Orange County Hair Transplant Surgery Office had a few concerns arise. Her concerns were very similar to those most women with hair loss or female pattern baldness would normally ask. “…how do I know if I’m a good hair transplant candidate?” “Is there any possibility that hair will fall off or … (affect) my regular hair to fall off?” “Do I have to stay at home after the surgery?” “Is there any bad effect of this kind of surgery on any area of my body or health?”

The most common issue Women with female pattern baldness or hair loss have is there is significant miniaturization or thinning of the hair shafts. This makes the patient an unlikely candidate for hair transplantation. In regards to lowering the hair line, as long as the donor hair has not been affected by male or female pattern baldness hair restoration is possible.

Hair restoration surgery is a permanent surgical option. Patients with significant hair thinning or pattern baldness might experience some degree of shock loss, loss of native hair due to surgical interruption, but some patients with some degree of miniaturization may not experience this effect as heavily or at all. Recovery time is about the same for most patients and the procedure should not interfere with their daily tasks. Most hair transplant surgeons will make sure to provide patients with specific medications to speed up the healing process and control the most common side effects (swelling and redness) in the recipient and donor area. Concealing the surgery can be easily done by simply changing your hair style a bit or by using any form of cover up (hat, bandanna, etc.).

Hair transplant surgery does not have any negative effect on the rest of the body. There may be some temporary swelling and redness but the only real cause for concern is the medications used during and after surgery. We highly suggest informing your hair transplant surgeon prior to the surgery if you have any type of allergy caused by certain medications. Food allergies should also be mentioned prior to surgery as many offices, like ours, provide an in office lunch. Hair transplant surgery is a very minimally invasive procedure and any negative effects to the body are considered unlikely and uncommon.

Scar Revision for Hair Transplant Surgeries

Scar revision for hair transplant patients applies to the correction or minimizing of the scar that was left behind by their hair transplant by strip procedure. One patient from a different clinic brought up this topic when he asked about correcting one side of the scar that got relatively wider. “Is it better to do only half and not touch the good side so to speak?”

When it comes down to hair restoration, there are a couple of options available for almost given situation including scar revision. The most common technique used is a simply removing and reclosing the scar itself. This alone can minimize the scar width. Some patients may even consider using BOTOX injections around the scar to prevent future stretching. Trichophytic closure is a special technique used to allow hairs to grow into the scar to further minimize the scar’s appearance and the last option available is an FUE (follicular unit extraction) procedure. With and FUE, we can selectively extract hair grafts and transplant them into the scar itself to eliminate scar visibility.

Scars can take anywhere from 6 or more months to mature or heal completely. This timely phenomenon limits how soon scar revision can take place. We highly recommend in our clinics not to revise any existing months until at least 6 months after the scar was created. Libido Affected By Propecia Use

Propecia is the only hair loss treatment FDA approved to be safe to use for male pattern baldness. Many hair transplant surgeons have used and prescribed Propecia because of its effectiveness. Propecia is a safe product but some patients have experienced a few side effects. This has raised some concern in many of our patients whom are considering Propecia for the first time. Here are a few questions some patients have submitted:

  • Will I still be able to get my partner pregnant?

Propecia has no adverse effect on sperm count whatsoever. Although Propecia may decrease your desire to have sex, fertility is not affected.

  • How would I know if my libido was affected by Propecia?

Even if your libido has decreased, some patients have reported to continue using Propecia regardless of its adverse effect on their sex drive. In addition, most of these same patients have reported a progressive increase in their libido with continued use. You have the option to discontinue the use of Propecia if you do experience sexual side effects. Considering the above statement, you may not need to do so unless the effects are substantial. A placebo test on Propecia has shown that an increase in functionality occurred at the same rate as with those on the placebo pill.

  • Even after a hair transplant surgery, do I still have to use Propecia tablets?

Of course. Having a transplant does not mean that the rest of your hair will not fall out. The hair normally lost due to male pattern baldness will continue to recede while the newly transplanted hair stays intact. At least compared to Rogaine, Propecia does not make a patient experience what is known as catch up hair loss. This is a phenomenon in which hair follicles regained through the medical treatment are lost again in a short period of time.

Which Finasteride Tablet Works Best?

Finasteride has taken on many different names in the market today. Some of those names include Proscar, Propecia, Fincar, Finpecia, Finax, Finast, Finara, Finalo, Prosteride, Gefina, and Finasterid IVAX. They all share one thing in common: they are all finasteride. The question has been asked in our Hair Transplant Clinic of Orange County, “What brand finasteride is more effective?”

Finasteride is normally associated with the treatment of an enlarged prostate. The notable side effect of finasteride is its ability to help re-grow hair or slow the hair loss process in men experiencing male pattern baldness. It was later introduced as an FDA approved hair loss treatment supplement. As for the names associated, they are all just name brands but they all share one thing in common: they are all finasteride.

Proscar is the commonly prescribed version of finasteride as it comes in 5mg tablets. This helps with the main concern which is price since you can cut the tablets into 1/4 size decreasing the amount to approximately 1.25mg tablets. This amount is safe for use but 1mg tablets are more than sufficient for the treatment of hair loss in men. We do have to caution patients is considering the use of finasteride 5mg tablets especially if their spouse is pregnant or may become pregnant. Propecia Associated To Hair Shed

Orange County Hair Transplant Clinic recently received an email from one of our patients who was concerned about having experienced some hair shedding after taking Propecia. His concern is if the medication will continue causing hair shed and if there are additional side effects associated with Propecia. Also if having been “malnutrition” at the time and having a family history of hair loss might be the reason for his current situation.

Propecia is a hair loss treatment supplement that has been clinically tested and proven to aid in hair restoration and is the only FDA approved drug for safe consumption in males with male pattern baldness.  In some cases, an increase in hair shedding may be evident during the first few weeks or months.  Within or after 6 months of taking Propecia, hair growth should become more evident.  It’s highly suggested not to be discouraged so early in the treatment and not to expect results too early as well.  As for sexual side effects associated with Propecia, they have only been recorded in 1 out of every 100 patients tested and continued use has shown that side effects become minimal or non-existent.  If sexual side effects continue, you can take a lesser amount of Propecia or discontinue its use.

People with a family history of hair loss may be more prone to male or female pattern baldness than most others.  On the plus  side, when caught early the effects of hair loss can be prevented, slowed or corrected.  Malnutrition can cause some hair loss but that can only be attested to severe vitamin deficiency.  Just be sure to maintain a well balanced diet and vitamin deficiency should not be an issue.  If your body is lacking in vitamins, vitamin supplements can help reverse that.
